Supplemental Documentation

Supplemental documents must be submitted on-line. Once the application is submitted, a link to upload documents will be activated (lower right-hand side of your User Home Page). Supplemental documents are due by June 15th for a July exam and January 15th for a February exam.

The Authorization and Release form should be uploaded immediately after submitting the application as the application will not be acknowledged without it.

Please read the acknowledgement letter carefully as it contains specific information regarding the required supplemental documentation.
